List

The List field enables users to add multiple entries of related information within a single form section.

Each entry is structured using predefined entity types, such as dependants, family members, or project items, allowing for consistent data collection.

This field is especially useful when capturing repeating sets of information where the number of entries may vary between users.

How to Create a List

  1. Right-click on Form Content.
  2. Select Create.
  3. Choose List Field.
  4. Enter a field name (e.g. Please provide details for each of your children).
  5. Under Field Settings > Entity Type, select or configure the entity type that defines the items within the list. For additional instructions, continue to the Adding Entity Types section below.
  6. Click Save.

The form will display a list field with the specified label, allowing users to add multiple entries according to the defined entity types.

Setting Up Entity Types

Before you can use a List field, you’ll need to set up Entity Types.

An Entity Type is used to define what information will be collected for each item in a list. It acts as a template that ensures the same set of information is captured every time a new entry is added. 

For example, you may create an Entity Type called Dependent that includes details such as name, age, and date of birth. This allows respondents to add multiple dependents, with each entry following the same format.

Step 1 - Open the Entity Type Creation Menu

  1. In your form workspace, right-click Entity Types in the left-hand menu.
  2. Select Create → Form Entity Type.

Step 2 - Name and save your Entity Type

  1. Enter a name (e.g. Child's Dependent) to identify this group.
  2. Click Save.

Step 3 - Add fields to your Entity Type

  1. Expand the Entity Type you just created.
  2. Right-click on Entity (this appears under your new Entity Type).
  3. Select Create, then add the fields you need (e.g. Name, Age, Date of Birth).

Step 4 - Create a List field and link your Entity Type

  1. In your form workspace, right-click Form Content to add the field.
  2. Select Create → List Field.
  3. In Allowed Entity Types, choose the Entity Type(s) you created.

Step 5 – Save and Review

  1. Click Save to apply the List field configuration.

  2. When the form is previewed, the List field will be displayed according to the configuration defined in the previous steps, as shown in the example above.

  3. Where applicable, add a new item based on the selected Entity Type (e.g. Add Child’s Dependent) to confirm the field functions as intended.
